Severe Headaches no Meds working!

I have been suffering from migraine headaches since I was in 6th grade which was in 1992.  Back then I was not able to ride the school bus because I would get too hot and would not be able to do my homework.  I started seeing a neuroligst in the 8th grade because my migraines became so frequent (3 or 4 a week).  I have been on numerous medications.  I tried nortrptilyne (sp) for a while and it never did any good.  My neurologist put me on Topomax 4 years ago.  However, the pharmacy informed me that it decreases the effectiveness of my birth control.  (They told me a little late!  I have two children only 10 months apart!)  I have not been on topomax for almost a year now, but my headaches are not the same they are much more painful and they last anywhere from 2 days to the longest 16 days.  I woke up fine but as the day progress it continually gets worse.  By bedtime I am in so much pain laying down puts too much pressure on my head.  I am nauseated with these headaches.  Some things make these headaches worse, such as when I have to strain a little it makes it worse.  I can't bend over and I can't focus on anything really well.  My head hurts in the center to right.  I can put a circle around where the pain is located.  Any suggestions on how to prevent these headaches or what I can take to get rid of them?  The doctor has prescribed flexiril and mobic.  She says if these don't work then she will try another route.  Niether one of these medications are helping!  I can't even take something over the counter to get rid of these headaches!  HELP!
