Dear Doc(s),

Oct ’05 had a low risk encounter with unknown partner (where my precum got exposed to her vagina for about 30 seconds, and i dont recall having any other penentration; just handjob with lotion). I got tested at the six week interval and was fine.   No symptoms until April ’06 when I had to get treated for tonsillitis. No major probs after that. Low risk encounters in Aug ’06 and Dec ’06. In each encounter, I sucked the breasts of the girls and a hand job with lotion; in one case the girl had a two year old baby, so not sure if she was milking or not. I however do not recall seeing any visible milk or blood in either of them.

Sudden symptoms of white tongue (with bitter metallic taste) between May ‘07 and Aug ‘07 of last year. The first doctor although did not suspect thrush based on what he saw, but still put me on some anti fungal medications. It did not subside. A few days later, I was on a different set of antifungal medications, again to no avail. Few days later, I went for a third medical checkup where they did tests for yeast and other things. It turned out okay, but blood test showed a little high sugar value (nothing to be concerned of as per doctor). After Aug of ’07, all the tongue problems subsided, and was good for four months until Dec ‘07.

Jan ’08 - again, occurrence of white tongue started, and still persists in its eight month.

Last 1month, sudden occurrence of frequent urination symptoms.  Doc ran tests for UTI & sugar in urine, but nothing turned out. However, when a different set of doctors did a test for Mycoplasma, they found both Aerobic and Anaerobic bacteria. I have no idea what they are. I took antibiotics for both.  However, the urination symptoms still persist, and so does the white tongue although it's not painful.  

Also, i read somewhere that both frequent urination & white tongue for extended durations could be yeast related.  Should i retest for yeast, UTI and STD's?

Nothing in your history suggests that you have had exposures which put you at risk for HIV or other STDs, nor do your symptoms suggest STD.  

The white tongue problem is unlikely to be a yeast infection.  You have been tested by several doctors who wondered about yeast but found no evidence. That you did not have yeast was verified by the fact that treatment directed at yeast infection did not help.  There are many other dermatological processes which can cause yeast. I would suggest seeing a dermatologist for diagnosis.  (And, as a side note, yeast infections only become a major problem for persons with HIV who have been infected for many (>10) years.  You know that is not the case, you've had a negative test).

As for the urinary frequency, your tests tell you that you do not have STD or UTI.  Mycoplasma species are difficult to grow but studies have shown that they are present normally in the penis is normal, non-infected men.  For your burning on urination problems you will need to work on it with your doctor.

Bottom line- there is nothing in your history to suggest that you have an STD or HIV.  Please do not worry about this possibility.  EWH

Thanks for your response.  As much as it is some relief to hear from you, but I just wanted to put in some additonal notes which i could not in my earlier request.   I sincerely appreciate your time in going through these.

Actually, the mycoplasma related tests have not yet been performed due to the fact that there was some aerobic and anaerobic bacteria found in my semen. This was during a semen analysis that was done last month as part of an IUI procedure that me and my wife initially planned.  The two drugs that were given for the treatment are "Doxycycline" and "Metronidazole".  The doctors told me that only after they could treat the infections with the above drugs, they can proceed to do a mycoplasma test.  

But,  now with all the other STD related thoughts in my mind,  I am hesitating to go in for the IUI procedures, until things are clear.

Also,  I dont have burning sensation as part of my frequent urination symptoms; its just the sudden onset of this frequency i am worried about in conjunction with the white tongue symptoms.

So, i'm kind of worried and axious as to what is related to what thats going on.

Could u please advise?

Thanks Doc.  I however went ahead and got tested with the FDA approved Home Access test kit, and got back my results which are fine. (Thank God !!!)
However, i had one last question - the test kit mentions it tests only for hiv-1.
Would u know if the same test also would have tested for the lesser rare hiv-2 OR should i not be concerned?

You do not need to be concerned.  If you had HIV-2, the test would detected it, either as a positive test of an intedeterminate result.  The evaluation of the indeterminant result would have then led to the correct diagnosis.  It is now time for you to forget your risk from the exposures you described because there were none and move forward with your life.  EWH
