carotid desease

51 year old male
Family history carotid desease
Stats:
(brother has stroke at 65 year old - almost dies and now is in wheelchair, 86% blocakge in CT artery)
excellent health (BP rest 50)
excellent diet and regular excercise (aerobics and weight trng)
6 ft 175 lbs
BP normal (except I have white coat syndrome so it always high at Doc's office)
Total Cholesterol range 130
HDL 43
LDL 66
trigl 130
lip a being tested
homocystine 9.1
CRP .3
carotid dopler impression;
focal areas of very mild homogeneous plaque are present bilateraaly. no hemodynamically significant
stenosos are present. No change from my last test.
had echo and treadmill stress test and everything is excellent

Do you agree with starting niaspan at 500 dosage?

What would you do

Niaspan sounds like a good choice. Im not sure about your question of what to do? You have plaque within your blood vessel but no narrowing. There is no indication for surgery or intervention at this point. You should have aggressive control of your cholesterol. Your HDL is low. Your weight is appropriate. I would look to eliminate animal fat from your diet. Your body wont make cholesterol if you dont eat fat. Aim for total fat <35 gm/day and sat fat<12 gm/ day, non fat milk. etc. this is a proven diet for plaque regression
