causes for low digoxin level

my 23 yr old daughter was started on lanoxin two months ago after the recall on digitek...  the day before she started lanoxin she had a dig level drawn and it was low,  she had been taking two 125mcg tab four times a week with only one tab the other days...  that amount was increased to two tablets every day....  subsequently she had another dig level drawn and it was still low and now she is taking three tablets mon, wed and friday...   i made sure when these levels were drawn that we waited 8 hrs after taking the tablets...  i am wondering what would cause this...  she also takes 25 mgs 2x day coreg, 2.5 mg enalapril, 25mg inspra for her heart and 300 mg seroquel, 25mg lamictal, and 100mg strattera (these are psych meds as she is developmentally disabled with mood disorder)   she had never had a metabolism issue with digitek      also... could lanoxin cause her become more fatigued??   over the last three months she has been taking about an hour and half nap when she comes home from her day program and she hadn't been for quite some time (i am waiting for her ef number from her last echo to come back, she has dilated cardiomyopathy that may be due to arrythmia)

Well the usual maintenance dose is 0.25 mg/day, unless you are very old or have renal disease.  It seems that she is on a lower dose for some reason, which may be fine.  My level is below therapeutic as well but my SVT is pretty much controlled on 0.125 mg/day, so we don't care.  The Coreg at her dose is probably responsible for her fatigue or the Seroquel, but not the Dig.
