He just won't eat anything different

My son was born with severe reflux and asthma.  He has been in and out of the hospital to many times to count.  He has had 6 surgeries which included a Fundoplication done 3 years ago in order to prevent reflux.  During that surgery, they also gave him a Mickey Button so he could receive formula.  He was given the ok to eat anything except citrus, nuts and caffine.  He is now 7 and weighs 41 lbs, he gets 5 cans of Nutren Jr. each day because we cannot get him to eat anything new.  His food includes: Ramen noodles (chicken only with the broth strained), cheese pizza, chicken nuggets (Wendy's or McDonalds only), spagetti (no meat or veggies), french fries (McDonalds), Doritos, Cheetos, Goldfish, grapes, peeled/sliced apples and white bread (no crust).  We have tried everything.  He understands how bad it is on his body to not eat and how it effects him physically (his immune system is very low), but he says "something" won't let him eat.  Is it just fear?  Does he somehow remember all of the pain he had when he was small?  What do I do?

The Nutren Jr gives him the nutrients he needs to grow which is good. Maybe engage him with helping you in the kitchen with making food and setting the table, etc. Make him milkshakes and have him make them with you. He may very well remember the pain from when he was young. Suggest you have him evaluated for child behavior with eating to help him with that.
