your blood pressure depends on a lot of things. Firstly, you should determine if your activities have changed in these 3 weeks. (maybe your exercising intensity as increased or your diet has changed). Although 146/80 (+/- 3 points) does not seems to me a lot of hypertension, but because your have a heart problem you should check with your physician as this could a sign of something.
